I recently wrote about my issues with cpu occupancy >99% in these forums but I can't find it to reference it. It turned out to be my CPU was overheating and thus it was being throttled back until it cooled down. I didn't have to replace the CPU - I removed the heatsink and cleaned and renewed the thermal paste. If your PC's CPU hasn't had the thermal paste replaced in over 5 years it is possible its condition has deteriorated.
It is also worthwhile ensuring all the fans in your system are cleaned occasionally and dust removed from heatsink fins because that air has to flow...
Your PC specs are higher than mine, so if you've tried reducing graphics settings and that hasn't helped, make sure your CPU temperature is stable and not cycling up and down.
